Find Me Five - Teen

About this form:

This form is intended for those in grades 9-12. Fill in as much of this profile as you wish. The more information you provide, the more likely we can suggest books you will like. Leave areas blank where you have no preference. Irving Public Library staff will use your answers to create a customized reading list of books to put on hold for you. Once the holds are filled you will receive notification from the library that your holds are ready to be picked up at the location of your choice. Information will be kept in strict confidence between the patron and library staff. You will receive a response within approximately 48 hours, Monday through Friday.
